Accepted and Current Nursing Students
There are a number of steps you must take before taking classes as a SUNY Delhi Nursing Student. Due to Federal, State and clinical site mandates for healthcare providers, there are more health requirements for nursing that the rest of the college community. Please read the following information carefully.
- Get a SUNY Delhi Health Form
- Schedule an appointment for a Physical
- Complete Required Forms
- Send all required forms by June 1st
- Present current CPR Certification card
Receive a SUNY Delhi Health Form

The Health Form is mailed to you from Counseling and Health Services.

An additional copy of the Health Form is also provided to you in your Accepted Student Day information packet.

If you did not receive the Health Form, you can download it online.

Your completed Heath Form must be submitted to Counseling Health Services by June 1st.
Schedule a Doctor's Appointment
- Schedule an appointment with your doctor for a physical.
- Be sure to have the doctor complete the SUNY Delhi Health Form and give you all of the required shots and tests.
- Take the SUNY Delhi Nursing Health Requirements list with you to your appointment, so the doctor can be aware of all the steps required for your clinical participation. Download Nursing Health Requirements list now >
- A complete physical and immunization record is required PRIOR to all clinical courses.
- Copies of all titers and immunization record need to be attached to completed Health Form.

Complete All Required Forms
- Complete the Release of Information Request. Download & Print Now >
- Submit form along with your Health Form to Counseling and Health Services.
Submit Required Forms before June 1st.
- Submit your completed Health Form and Release of Information Form to Counseling and Health Services.
- Copies of all titers and immunization record need to be attached to completed Health Form.

- Any nursing student who has not submitted all required information prior to June 1, will be de-registered from nursing courses.
Obtain CPR Certification prior to first day of classes
- All nursing students must be certified in CPR - Professional Rescuer / Healthcare Provider prior to attending the first day of classes.
- Proof of certification in CPR for the Professional Rescuer from the Red Cross or American Heart Association must be presented to Nursing faculty on the first day of classes. (front and back copy of your CPR card.)
- Bring this card with you to campus.
- Do not submit the card to Counseling & Health Services or to the Nursing Department. Submit the card to a Nursing faculty member.
- Contact your local Red Cross, American Heart Association, EMT, or hospital in your community to obtain information of certification programs.

HS Average: 80
Algebra Regents: 80
Biology Regents: 80
English Regents: 80
Transfer GPA: 3.0
