To submit a proposal, download the "Request for IRB Review" form (available on our Forms page), complete and send via email to Amber Tatnall, IRB Chair. Make 2 copies and send to one to IRB Chair, and one to the Provost's Office, Bush Hall. See Policies and Procedures for further instructions.Contact Info
