Forms & Instructions for Submission
The Access and Equity Services Office is going paperless. You must save the forms to your computer before you begin working with them. After the forms have been saved you will be allowed to complete and sign electronically using Adobe Reader. All completed and signed forms should be returned via email or fax to the Access and Equity Services Office.

Student Registration and Self Assessment
All students enrolled in programs (classroom or online) through SUNY Delhi need to formally indicate their disability status by completing and submitting this form.
Summer Transition Program Registration Form
Use this form to make a reservation for Summer Transition Day. Summer Transition Day is for accepted Students with Disabilities and their families to learn more about the support services offered through SUNY Delhi Office of Access and Equity Services.
Request for Text in Alternate Formats
Students who have Electronic Books listed on their Accommodation Letters may request copies of their textbooks in alternative formats. The college uses a variety of sources to supply these materials, including files obtained from the publisher and scanning books in house. In order to request texts in alternative formats, please complete this form and submit to the Access and Equity Services Office. You must submit a form for each book you are requesting. Once you have submitted a request you must also provide the Access and Equity Office with proof of purchase, i.e. receipts. The last step is to sign the E-Text Agreement which should be signed electronically and emailed to the Access and Equity Office.
If you have been accepted for admission at SUNY Delhi and would like to register with Access and Equity Services, please submit this form which was included in your acceptance packet.
Accommodated Testing Request for Instructor - Testing Slip
This form must be emailed to your teacher when you are requesting to take a test with accommodations. Be sure to send this form after you submit an online Accommodated Testing Notification to the Resnick Learning Center.
