All students, residential and commuter, taking 6 or more credit hours, must submit the following which are all part of the same form; Health Form: 

1.    Health History - the front of the form and parts A and B

This includes required immunization information--NYS Public Health Law section 2165 requires students, born after January 1, 1957, who attend institutions of higher education in New York State, to provide documentation of having received two MMR (measles, mumps, and rubella) vaccinations.  For more specific details about this requirement contact us at healthservices@delhi.edu or 607-746-4690.

2.     Tuberculosis Screening Form 

3.     Consent of Parent or Guardian for Medical or Emergency Treatment Form--for students under the age of 18

 Programs with special Admission Requirements 

Nursing students and athletes have other requirements.  Refer to the nursing department or the athletic department for more information.
