Volunteer Application and Waiver
Request for Volunteer Hours (RVH) - print out, fill out and bring to office. Please allow for 7 - 10 day turnaround. We will process your request as soon as we can and will return the results to your campus mailbox.
Online Summary of Community Service for faculty and staff only - fill out online, and then email the office to notify us of your submission. Due to limited staffing, we are unable to verify students submissions at this time. Thanks for your patience while we transition to paperless forms.
