The deadline for residential students to request exemption waivers for the COVID-19 vaccine has passed.
No requests for waivers will be accepted for residential students at this time.

COVID-19 Religious & Medical Exemption Waiver Requests
for Non-residential Students

The deadline for residential students to request a medical or religious exemption was Monday, August 16.

For non-residential students, there is an open 10-day period, from August 24 through September 3, to apply for an exemption. Exemption requests will be reviewed at least once per week during this time with decisions sent to the students via their SUNY Delhi email. Applying for a COVID-19 exemption does not allow an extension to the September 27 deadline for vaccination if the exemption is not approved.

Students who have not demonstrated proof of a completed COVID-19 vaccination series or who do not have an exemption approved by SUNY Delhi by September 27 will be disenrolled from their courses and the college will follow its standard tuition refund schedule.

Follow these directions to upload completed waivers to the patient portal: 

  1. Print out and complete the form.
  2. Scan or take a photo of the completed form.
  3. Sign in to the SUNY Delhi patient portal.
  4. Click on the upload link on the menu bar at the top of the page and follow the step-by-step instructions.
  5. Once uploaded, the waivers will be stored in your immunization record.
  6. Waivers will be reviewed and are subject to approval.
