If you have been accepted for admission at SUNY Delhi and would like to register with the Office of Access and Equity Services, please submit this form to Connect with Access and Equity Services This form was also included in your acceptance packet.

Student Registration and Self-Assessment

All students enrolled in programs (online or off-site) through SUNY Delhi need to formally indicate their disability status by completing and submitting the Student Registration and Self-Assessment Form. Students enrolled in programs on the SUNY Delhi campus DO NOT COMPLETE this form, but must make an appointment to meet with the Access and Equity Coordinator as soon as possible. Summer Transition Program Registration 2021

Use this Summer Transition Program Registration Form to make a reservation for the 2021 Summer Transition Program. Summer Transition is for accepted Students with Disabilities and their families to learn more about the support services offered through SUNY Delhi's Office of Access and Equity Services.

Request for Text in Alternate Formats

Students who have Electronic Books listed on their Access Letters may request copies of their textbooks in alternative formats. The college uses a variety of sources to supply these materials, including files obtained from the publisher and scanning books in house. In order to request texts in alternative formats, please complete the Request for Text in Alternate Format which can be found in Vancko Hall. You must submit a form for each book you are requesting. Once you have submitted a request, you must also provide the Access and Equity Office with proof of purchase, e.g., receipts. The last step is to sign the Copyright Agreement which should be signed electronically and emailed to the Access and Equity Services Office.

If you have special housing needs, you will need to document those needs. Please review the accessible housing guidelines and complete the appropriate forms below.
