Accommodated Testing Form

Please fill in the form below with the testing instructions you want the proctor to follow. Once completed, attach a copy of your exam below. If there are any issues, please contact test@delhi.edu or ext. 4740. 

Remember: All tests must be submitted 48 hours prior to the exam.
